CONFIDENTIAL CLIENT CHECKLIST
Be prepared to provide valid identification for the taxpayer and spouse, Social Security or ITIN information for everyone included on the return, dates of birth and your Identity Protection PIN if one was issued by the IRS.
New clients may also be asked for a copy of the previous year’s federal and state tax returns. Additional documentation may be needed to establish a qualifying child, dependent or household member.
Gather all income statements received for the tax year. These may include Forms W-2, 1099-NEC, 1099-MISC, 1099-K, 1099-INT, 1099-DIV, 1099-B, 1099-R, SSA-1099 and 1099-G.
Depending on your circumstances, we may also request records for rental income, digital assets, gambling winnings, alimony, retirement distributions or other income not reported on a traditional wage statement.
Relevant documents may include childcare expenses and provider information, Form 1098-T for education expenses, student-loan interest, mortgage interest, property taxes, charitable contributions and qualifying medical expenses.
We may also ask about estimated tax payments, energy-efficient home improvements, eligible vehicle purchases, retirement contributions and other transactions that could affect the return.
Self-employed taxpayers should prepare complete records of gross income, business expenses, payment-processing activity, business bank statements, Forms 1099, inventory and equipment purchased or sold during the year.
Additional information may be required for business mileage, home office use, contractors, employees, vehicle expenses, startup costs, business loans or assets placed in service.
Please provide Form 1095-A if health insurance was obtained through the Marketplace. You should also provide any IRS or state tax notices, amended-return information and records involving property sales, casualty losses, inheritance or other significant financial changes.
Major life events—including marriage, divorce, the birth of a child, relocation, homeownership or changes in dependent custody—should be discussed even when no specific tax form was issued.
